CORE REQUIREMENTS FOR PASSPORT RELEASING

1. Original issued receipt;
2. Old Passport (for renewal applications);
3. Authorization letter (If applicant is unable to claim his/her passport)

For minor applicants

• If passport will be claimed by either mother or father, claimant must present original issued receipt, passport and PSA-issued documents indicating proof of parental authority over the minor.

Note: Passports unclaimed after one (1) year will be cancelled per Department Order No. 2021-012.
