Requirements for Report of Marriage
Bring the original and four (4) copies of the following:
- Valid passports of the spouses;
- Personal appearance of both parties are required;
- If married in a religious ceremony, the Marriage Certificate should be translated to English and authenticated by Israel’s Religious Affairs, and then authenticated by the Ministry of Interior;
-- For Filipino nationals
---Original Birth Certificate issued by of the Philippine Statistics Authority (PSA) and apostilled by Department of Foreign Affairs;
--- Original Certificate of No Marriage (CENOMAR) issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (not more than six months old).
-- For Israeli nationals
--- Birth certificate issued by the Ministry of Interior and apostilled by MFA;
--- Extract/Abstract of Registration issued by the Ministry of Interior and apostille by the MFA.
- For those annulled/divorced and widowed applicants, additional requirements will be requested (see list of supporting documents).
Notes for Marriages Solemnized outside Israel:
- The Embassy will only notarize the Report of Marriage. The applicants will forward the Report of Marriage and all supporting documents to the Philippine Embassy in Athens (for those married in Cyprus) and/or to the Philippine Embassy in Ankara (for those married in Georgia) for recording.
- Cyprus is under the consular jurisdiction of the Philippine Embassy in Athens and Georgia is under the consular jurisdiction of the Philippine Embassy in Ankara.
Suppporting Documents
For Annulled/Divorced Marriages:
- Apostilled PSA issued Marriage Certificate with annotation that the marriage was null and void
- Apostilled Court Decision or Finality
For Widowed:
- Apostilled PSA issued Marriage Certificate
- Apostilled PSA issued Advisory on Marriages
- Apostilled PSA issued Death Certificate of spouse
Note:
- Additional documents maybe required depending on the circumstances of the applicant.
Fees:
- $25.00 or its equivalent amount in NIS based on current Embassy exchange rate
- Additional $25.00 or its equivalent for affidavits that maybe executed
