- Original Report of Birth (ROB) issued by the Embassy or Birth Certificate issued by the Philippine Statistics Autority 

- Original and Photocopy of Passport of either Parent

- Personal Appearance of Minor Applicant and either parent or authorized adult companion

- Photocopy of PSA Marriage Certificate or Report of Marriage of Parents issued at the Embassy (for legitimate child)

- School ID (if available)

- For minors born abroad and above age of majority, Certificate of Exemption for IDF duties

Notes:

- Passport of Minor Applicants must be claimed by either one of his/her legally registered parents who must be married. Only the mother may claim the passport if the father is not legally married to the mother.

- Passport of Minor Applicants cannot be claimed by any authorized representative.

- Embassy personnel may conduct further verification of authorized claim for security reasons.

Pursuant to Department Order No. 37-03, all passports unclaimed by applicants after six (6) months from the date of issuance shall be cancelled.

Travel Document (Minor)

1. Duly accomplished Assistance to  Nationals (ATN) Request Form
2. Duly Accomplished Passport Application Form
3. Report of Birth (registered/issued by the Philippine Embassy – 1 set)
4. Itinerary of Travel (Ticket or Booking Reservation) of applicant and travelling companion
5. Three (3) pieces passport-size photos of applicant
6. Copy of Passport of Travelling Companion
7. Affidavit of Support and Consent (if not travelling with parent)
8. Affidavit of Explanation (if applicable)

Travel Document (Adult)

1. Duly accomplished Assistance to  Nationals (ATN) Request Form
2. Duly Accomplished Passport Application Form
3. Passport
4. Itinerary of Travel (Ticket or Booking Reservation)
5. Three (3) pieces passport-sized photos of applicant
6. Original PSA Birth Certificate duly authenticated by DFA or red ribbon
7. Original PSA Marriage Certificated duly authenticated by DFA or red ribbon (if married)
8. Valid IDs if applicant is a holder of Green or Brown passport.

           Additional USD 25 for the notarial (affidavit)

Notes:

- A Travel Document may only be issued for emergency/humanitarian reasons.

- Date of travel must be within the next thirty (30) days from issuance date of the Travel Document.
