Report of Marriage

Marriages Solemnized in Israel:

Bring the original and four (4) photocopies of the following:

  1. If married in a religious ceremony, the Marriage Certificate should be translated to english and authenticated by Israel’s Religious Affairs, and then authenticated by the Ministry of Interior;
  2. Original Birth Certificate issued by of the Philippine Statistics Authority (PSA) and apostilled by Department of Foreign Affairs;
    • For Israeli Nationals birth certificate issued by the Ministry of Interior and apostilled by MFA;
  3. Valid passports of the spouses;
  4. Original Certificate of No Marriage (CENOMAR) issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (not more than six months old)
    • For Israeli Nationals Extract/Abstract of Registration issued by the Ministry of Interior and apostille by the MFA.
  5. Report of Marriage (form available at the Embassy);
  6. Personal appearance of both parties are required.

 

*The requirements apply to marriages (1) between Filipinos, and (2) between a Filipino and a foreigner solemnized in Israel.

 

Marriages Solemnized in Cyprus:

Bring the original and four (4) photocopies of the following:

  1. Original Marriage Certificate issued by the solemnizing officer in Cyprus
  2. Required documents submitted to the solemnizing officer in Cyprus such as Certificate of Legal Capacity to Contract Marriage of both applicants, copy of passport used to travel to Cyprus, etc.
  3. Original Birth Certificate of the parties authenticated/apostilled by DFA/MFA;
  4. Valid Philippine passports of the spouses;
  5. Personal appearance of both parties are required;
  6. Original Certificate of No Marriage (CENOMAR) issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (not more than six months old)
    • For Israeli Nationals Extract/Abstract of Registration issued by the Ministry of Interior and apostille by the MFA.

 

*The Embassy will only notarize the Report of Marriage. The applicants will forward the Report of Marriage and all documents to the Philippine Embassy in Athens for recording. Cyprus is under the consular jurisdiction of the Philippine Embassy in Athens.

 

Fees: Report of Marriage:

Registration Fee for Report of Marriage US$ 25.00 (or its equivalent amount in NIS based on current Embassy exchange rate)
