Requirements for Processing of Philippine Overseas Employment Administration (POEA) Standard Employment Contract of Household Workers for Diplomats

 

  1. Two copies of the contract ( original and photocopy ) - duly signed by the employer and worker may sign contract during processing at POEA;
  2. Two copies of passport or ID of the employer;
  3. Two copies of the Employer's letter to the Israel Ministry of Foreign Affairs for the issuance of visa to the worker;
  4. Letter-request to the Philippine Overseas Labor Office (POLO) for the processing of the contract with an explanation how the worker was introduced to the employer;
  5. Two copies of worker's passport;
  6. Fees:       US$ 12.00 for verification

                    US$ 25:00 for authencation
